Failed Validation After Remediation
A remediation job completed its steps, but the post-fix validation check did not confirm the vulnerability or misconfiguration was actually resolved.

- Trigger
- A remediation job's validation step returns a failure after the job's action steps reported success (AIQ-RMD-005).
- Risk
- Without successful validation, AssurIQ cannot confirm the underlying risk was actually reduced, even though the job ran. Treating an unvalidated fix as complete would produce a false sense of assurance.
Prerequisites
- Confirm the job's step-by-step execution log to rule out silent partial failure
- Confirm whether a pending reboot or service restart is required for the fix to take effect
- Confirm the target's evidence source has re-synced since the fix was applied
Recommended fix
- Open the job's step logs in Fix Center and confirm every action step actually completed successfully.
- Check for a pending reboot or service restart that would prevent the fix from taking effect until completed.
- If a reboot is pending, schedule it and re-run validation afterward.
- If the evidence source had not yet re-synced, wait for the next collection cycle and re-run validation.
- If validation still fails, inspect whether the patch was superseded or the configuration reverted by another process.
- Re-run the remediation from the failed step, or roll back if a partial change left the target in an inconsistent state.
Validation
- Re-run validation returns a pass
- Vulnerability or misconfiguration re-scan confirms the underlying issue is resolved
- Job status in Fix Center shows Validated
Rollback and recovery
- If repeated validation attempts fail and the partial change is disruptive, roll back the job to restore the pre-change state.
- Re-open the remediation item for re-planning after rollback.
Evidence generated
- Step-by-step job execution log
- Validation attempt history with pass/fail results
- Final re-scan or configuration confirmation evidence
